Free change order form
Fill in the form below, then print it or save it as a PDF and have your client sign it before you start the extra work. The new contract total is worked out for you. Everything stays in your browser: nothing you type is uploaded or stored.
How to use this form
- Fill it in before the extra work starts. A change order signed after the fact is much easier to argue about.
- Use one form per change and number them (#1, #2, ...), so nothing is billed twice.
- Always show the new contract total. Clients approve small extras one at a time and are then surprised by the sum.
- Save as PDF: choose "Print or save as PDF", then pick "Save as PDF" as the printer. On a phone, use the share or print option in your browser.
- Give the client a copy of the signed form and keep one yourself.

This form is a general template, not legal advice. Some places have specific rules for home-improvement contracts and change orders (California, for example, requires them in writing and signed before the work starts). Check the rules where you work, or ask a local construction attorney or your licensing board.
Skip the printer: get it signed on the client's phone
We're building ChangeOrderSignoff: write the change order on site, text the client a signing link, and both of you get a signed PDF with a timestamp. A flat $19/month for your whole crew, with a 14-day free trial. It isn't launched yet; join the waitlist for 50% off for life.
